browser/OS plugin that told me where my tabs came from
which application or person or Twitter account/etc, even days later.. maybe a little screenshot showing where the link was that opened it

This is exactly one of my frustrations! I end up having dozens of tabs open, and I don't always remember where they came from. I started building a Firefox plugin last year that tracks the tabs that open, and the referrer of each page that loads. The idea is to then map the "web" of connected pages. I can tag pages as I go along, to note the topic or reason I'm on the page. This is particularly useful when I go on "tangents" because good articles link to other resources that take me "off topic".
I've come across some tools that are similar, but not the same. More similar tools keep popping up, but not in this way yet. Would love to know if this is something you have seen somewhere already or are working on.
I would love to have this be something like the Awesomebar that works behind the scenes and compiles history from all browsers (idea noted somewhere else on this forum).
